Background Screenings
All Club staff, including Board Members, Team Manager and Coaches, MUST have a current Background Screening. All screenings will be conducted through the USA Hockey National Office. The link is below.
SAFESPORT TRAINING-Board Members/Team Managers
All Club staff, including Board Members, Team Manager and Coaches, MUST complete SafeSport Training this season. Players who turn 17 or older in 2021 must also complete the training. Once training is complete each club must submit a single email with the SafeSport certificates of completion for all Board Members and Team Managers who do not appear on a team T1 Roster. Certificates do not need to be submitted for Coaches or Players as their completion will be audited by USAH and they cannot be rostered unless the requirement is met. PARENT CODE OF CONDUCT & WAIVER
The AT&T MHSHL Parent Code of Conduct, AT&T MHSHL and COVID-19 Waivers are included when parents register their son/daughter with the league. You no longer need to complete these forms separately.
CLUB DOCUMENTS
Each of the following documents must be completed and returned to the league. The Club Information Form and the Club Participation Agreement should be returned immediately. The Club School Attendance Form should be returned by September 1. The Academic Eligibility Form Should be submitted each grading period during the season when player(s) do not meet the pass/fail criteria.

LINE-UP CARD
The Line-Up Card is to be used to submit your line-up each game to the scorekeeper. You should list all rostered players and strike through any player that is not playing with a single line. This is the best way for the scorekeeper to track the players actually playing in that game.
